Report a pothole
A hole in a city street goes to 311 — call 574-233-0311, or dial 311, from 7:30 am to 5:30 pm, or use the online form. Give the block and which side of the road, and say whether it is one hole, several, cracking or just rough pavement. If a pothole damaged your car, the city points claims at its Legal Department on 574-235-9241.
The Pavement Condition Index runs 0 to 100 and comes from measured surface damage — cracking, potholes, rutting, patching. The seven bands below are the standard ones (ASTM D6433) used by pavement surveys across the country, so a 62 here means the same as a 62 anywhere else.

The Streets & Sewers division maintains most streets inside the city limits, but not all of them. State Road 23 — South Bend Avenue, Eddy Street, Sample Street between Eddy and Chapin, Chapin south of Sample, and Prairie Avenue — and State Road 933 / US 31 along Lincoln Way East belong to INDOT. Report a pothole on those to 1-855-463-6848. Alleys are handled separately again.
City crews sweep every city-maintained street neighbourhood by neighbourhood in the spring once the snow thaws, and again in late fall after the ReLeaf leaf collection finishes. A city-wide sweep takes one to two weeks depending on how many holes there are. Through the winter the fill is a temporary cold patch, because the pavement is too cold to hold anything else; the more permanent hot patch goes in once the asphalt plants reopen in spring.
The survey scores a block and recommends a treatment for it. What actually gets done each year is set by the city's paving budget and programme, which covers a fraction of the network. A block can sit at a low score for several years with the same recommendation beside it. The paving map on the city's Streets page is the place to see what is funded.
Every score on this page is from a survey on a particular day, and each block says which day. A pothole that opened after that day is not in it. This page is a record of a survey, not a live picture of the road.
The recommended treatment is what the survey’s engineers judged the block needed at the time. It is not a scheduled job, not a budget commitment, and not a promise that anything will happen.
Not every road in town belongs to the city — state highways and county roads are surveyed and repaired by other authorities and may be missing here.
