Which way depends on what you are going for. The town's own development plan says it plainly: 「本町の住民の生活圏・経済圏においては、行政圏を異にする岡山県玉野市に属していると言っても過言ではなく、水・電気・学校・医療機関・商店など多くの恩恵を受けており」 — for daily life, Naoshima effectively belongs to Tamano in Okayama. Kagawa, three times further away, is where the prefectural administration and the big hospitals are.

Shikoku Kisen runs no car park at either end: 「専用駐車場はございません。周辺の駐車場をご利用ください」, and it says space is limited. If you keep a car on the mainland, the monthly options below are the reason many islanders do.
Tamano Hospital (たまの病院)
The mainland hospital nearest the island, formed in 2025 by merging the old Tamano City Hospital with Tamano Mitsui Hospital. A designated emergency hospital (救急告示病院) with night and emergency cover. Reception 8:30–11:30 and 13:30–16:00; closed Saturdays, Sundays, public holidays and 29 Dec–3 Jan.
Marunaka Uno (マルナカ宇野店)
Supermarket, a short walk inland from Uno station.

Shikoku Kisen (四国汽船). Return prices, doubled from Shikoku Kisen's published one-way fares and read on 8 August 2026. The company revised the Takamatsu fares on 1 June 2025 and the Uno fares on 1 February 2026 — the first Uno increase in over forty years — so these are the current figures. Child fare is 小学生, ages 6 to 12; one preschooler travels free with each adult and the second pays child fare. Bicycle and vehicle fares are charged on top of the passenger fare, and only on the car ferry. No route takes reservations: 「ご予約及び、事前販売は一切承っておりません」.

Shikoku Kisen (四国汽船). Return prices on the Takamatsu route, doubled from the published one-way fares and read on 8 August 2026. The high-speed boat is more than twice the ferry. Bicycles and vehicles go on the ferry only, and their fare is on top of the passenger fare.

There is no operator car park here either. The nearest is the Sunport underground car park beneath the symbol tower.
Kagawa Prefectural Central Hospital (香川県立中央病院)
Outpatient reception 8:15–16:00 with an appointment, 8:15–11:00 without. Bring a referral letter (紹介状): without one there is a first-visit surcharge of ¥7,700, and from 1 April 2026 the hospital applies it more strictly, including to some ambulance arrivals not judged a true emergency.
Takamatsu Red Cross Hospital (高松赤十字病院)
Same ¥7,700 first-visit surcharge without a referral, waived with one. Your doctor's practice can book you in by phoning or faxing the hospital's 地域医療室.
Kagawa Prefectural Government (香川県庁)
Open Monday to Friday, 8:30–17:15. About two kilometres inland from the pier — not a walk from the boat like the shops are.
Marunaka Sunport (マルナカサンポート店)
Supermarket directly behind JR Takamatsu station, so effectively next to the berth.
